The official box art for Dark Souls 2 was released by Namco Bandai this weekend. Like the covers for Dark Souls and Demon's Souls, it's a very minimalist depiction of the game.
In the box art, a lone warrior is walking toward a fog gate. Fog gates are passable walls that separate different sections of the game. They also appear before boss fights. These walls can also prevent you from leaving if another player has invaded your game, forcing you to face them.
The hero depicted, a sword-wielding knight with fur-lined armor, should look familiar by now. He's also depicted in the CGI trailer and the first gameplay footage. The game allows players to create their own custom character, though, so your player won't necessarily look like that.
Dark Souls 2 is in development for the Xbox 360, PS3 and PC. From Software insists that the PC version will be a lot better this time around.
